For social workers, GPs, memory services, occupational therapists, community teams and other professionals. Fill in what you know and a Care Manager will come back to you, usually within one working day. If anything is urgent, please call us on 01225 282477.

What happens next
- A Care Manager reviews the referral and contacts you.
- We visit the family at home to talk through the service.
- If it is the right fit, we match a Life Story Worker and arrange an introduction visit.
